Please submit a ticket in the Rider Care form, if you would like to update your address/phone/e-mail or BSN.
In case of BSN change or address change, submit government issued documentation such as: national ID card, excerpt from the population register (you can request this at your local muncipality).
In case of an address change, submit a document stating your new address (Not older than 6 months) such as: phone bill, health insurance bill or another (government) official document.
